VNA Healthtrends is excited to work with Prime ACO to impact patient outcomes, reduce re-hospitalization and decrease cost of services while maintaining strong clinical outcomes. Our team works closely with the patient’s physician and healthcare team to provide medical, social and environmental support to treat the whole patient. Reinforcing the importance of follow-up and compliance with physician visits places an increased level of importance on maintaining these appointments to the patient and family.
